Legacy of Life Hawai‘i (LLH) partners with hospitals and healthcare professionals across the state to support organ, eye, and tissue donation. Together, we share a responsibility to honor patients and families while saving and healing lives through donation.
Donation is a 24/7, 365-day responsibility, and our team is always available to support you.
Clinical Services
Our clinical staff is available at all times to facilitate the donation process.
Our Procurement Transplant Coordinators:

Work alongside hospital teams to maintain organ function

Coordinate with the United Network for Organ Sharing (UNOS) for fair organ allocation

Manage the surgical recovery process
A single organ donor can save up to eight lives through the donation of the heart, lungs, liver, kidneys, pancreas, and intestines.
In addition, tissue donation—including corneas, bone, skin, heart valves, and veins—supports life-enhancing procedures such as burn treatment, reconstruction, and injury repair. More than one million tissue grafts are performed annually in the U.S.

Please call LLH as soon as a potential donor is identified.
Our coordinators will:

Conduct a brief clinical assessment by phone

Respond on-site when appropriate

Collaborate with your team on next steps

Support family discussions when appropriate
Identifying a Potential Donor
Timely referral is critical.

Per CMS Conditions of Participation:
-
Refer all deaths and imminent deaths
-
Imminent death: ventilated patient with a Glasgow Coma Score (GCS) < 5
-
Refer within one hour of meeting criteria
-
For planned withdrawal of care, notify LLH prior to extubation
-
Refer all cardiac deaths within one hour of pronouncement
